More non ACS causes of troponin elevation

Just when you thought troponin couldn’t be less specific for ACS, additional non ACS causes of elevated troponin have been added to the Non ACS causes of elevated troponin. It is telling in itself that there is a whole page on this subject! So if you do troponins on patients at low risk of ACS, most of the results will be false positives. Having said that, someone did a troponin on an elderly lady who had had a simple fall last week (who also had very normal looking ECG) and it came back at 700 – so sometimes you just can’t win!
