IV steroids in severe CAP

The Management of CAP page has been updated with some information about the use of IV steroids in a selected population of patients with severe CAP. Bottom line is – sounds interesting, but not enough evidence currently to change practice.

EXTEND 1A Stroke trial

The Stroke Management, Thrombolytic therapy in stroke and Investigation of stroke pages have been updated following publication of the EXTEND 1A trial in the NEJM today showing substantial benefit for endovascular intervention following tPA in patients selected by CT perfusion to be likely to have benefit.

JVP measurement

The Cardiovascular examination page has been updated with a correction. The base from which the JVP is measured is the manubriosternal angle, not the sternal notch (although the base of the notch in a patient at 45 degrees is probably not far off).

Hyponatraemia flowchart errors

The Assessment of hyponatraemia flowchart on the Hyponatraemia page has the following errors: SIAD also commonly has low serum urate (not normal as on the chart); cerebral salt wasting syndrome usually has some degree of hypovolaemia (not euvolaemia as on the chart). The flowchart will be updated to correct these errors as soon as possible.
